Hours: Monday – Friday; Days and some evenings required to meet program needs; 7.5 hour shifts (Hours are subject to change based on operational needs)

Responsibilities:

The Halton Diabetes program provides diabetes education and management services for pediatric and adult patients. The Registered Nurse will:

  • Conduct thorough nursing assessments of both pediatric and adult patients with diabetes, taking into consideration ethno-cultural backgrounds and the social determinants of health
  • Develop and implement evidence based care plan for patients with with diabetes (Type 1, 2, and diabetes in pregnancy) and collaborate with the multidisciplinary team as needed
  • Use a variety of teaching methods and principles of adult education targeting different target population to engage with patients to enhance diabetes self-management in group and individual setting
  • Ensure continuity of care by maintaining up to-date medical records, communicating with the client’s primary care provider and other service providers, and making referrals where appropriate
  • Participate in program planning, evaluation and resource development based on a client-centered, inter-disciplinary approach.

Qualifications:

Halton Healthcare Services is seeking individuals who are competent in their chosen profession; patient/customer focused; team players; effective communicators; caring and respectful and are fiscally responsible.

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • BScN required
  • Certified Diabetes Educator required
  • Demonstrated application of the current diabetes evidence, adult education principles and self-management approaches through the lifecycle
  • 3-5 years previous experience in working with culturally diverse individuals and groups providing diabetes education and self-management support
  • Experience in insulin pump therapy and working with pediatric population an asset
  • Experience working effectively managing multiple competing demands in a variety of settings
  • Computer literacy (Microsoft Office)
  • Highly effective leader, decision maker, problem solver and excellent organizational skills
  • Excellent oral and written communication skills in English, speaking and writing in other languages an asset
  • Able to establish positive and supportive relationships with patients, families and other health professionals and represent the program
  • Able to create and deliver a cultural competent diabetes program for diverse ethno-cultural populations
  • Has use of a vehicle and a driver’s license
  • Able to meet the physical demands of the position

Satisfactory attendance and employment record required

Salary: $34.24 to $49.02 per hour
